Okay, so the only reason I mention this is because I didn't realize it for the longest time, but it could be due to how the knot is being tied (when you tie the bracelet to the wrist, that is.) Here is how to tie a square-knot properly, I used to just cross the same-side over twice, instead of switching them, which makes for a much less secure knot.

Also, if you're braiding the ends and tying them on that way, I have found that three-sectioned flat braids aren't as strong as things like four- or six-part round braids, like this. I know that link is or a dog leash with thicker rope, but the principle is the same and I wanted a link with pictures. :) My bracelets used to fall off other people and myself all the time, but I haven't had that problem since I changed my braids and knots.

If you're doing the buckle thing as described on this site, the big knot could be too small or the loops you put it through could be too big, etc. Or the bracelet might need to be adjusted so it is tighter, because the knot will go back through the loops on its own much easier if given the room to do it.
